Our doctors are providers for several PPO insurance companies. We require that co-payments and deductibles be paid at the time of your visit. The patient is primarily responsible for the account. We will file insurance on all worker’s compensation claims. We require payment for services rendered at the time of your visit. In event that you receive emergency room care and/or have surgery, we will file your insurance. For your convenience we do accept MasterCard and Visa.
